Dr. Baldassarre is in-network with Medicare, Aetna, Blue Cross Blue Shield, Cigna, Optum, and UHC.

Out-of-Network Insurance

Dr. Baldassarre is an out-of-network provider for all other insurance companies. Most other insurance companies provide at least partial coverage for the assessment. It is the responsibility of the patient or the patient’s guardian to determine, understand, and confirm their benefits with their insurance carrier. At the conclusion of the evaluation, the patient or patient’s guardian will be presented with a superbill to submit for reimbursement.

• Forms of Payment

  For out-of-network patients, payment for services is due at the time of the evaluation. We accept cash, check, and credit card. We also can accept a debit card associated with a health savings account or flexible savings account.

• Evaluation Cost

  As cost varies based upon time required for service provision, our administrator will provide you with a specific value during scheduling.
